I offer two convenient ways for you to obtain services. You can either pay for services directly, or you can go through your insurance. To pay for services, you can pay by cash or check; to use your insurance, call me so I can verify eligibility and begin a service authorization.
TO PAY CASH : Fees for cash paying patients are on a per-session basis:
$150 per session
TO USE YOUR INSURANCE : We gladly accept most insurance, including the following:

Most PPO Plans
Insurance Information Required
In order for me to verify eligibility and coverage, we require the following information that is found on your insurance card:
* Patient's name, date of birth, and policy number
* Insured's name, date of birth, and policy number if patient is not the primary individual insured
* Look closely at the back of your card for a phone number for SA/MH (Substance Abuse or Mental Health) benefits
